Wondering if you can use your dental benefits with us? Below are some of the insurance plans we are commonly asked about. Because individual plans and benefits can vary, please contact our office or your insurance company if you have questions about your specific coverage.

In-Network Plans
We are currently in-network with:
- Delta Dental
- Anthem — Complete, Grid+, Anthem 300 & Anthem FEP
- Does not include Anthem Medicaid or Medicare plans
- Principal
- Ameritas plans that utilize the Principal network
- Ohio Medicaid — traditional black & white paper card only
- CareSource / Tri-State
Out-of-Network Plans
We can still bill many out-of-network PPO plans and apply your available out-of-network benefits. Coverage and reimbursement vary by plan.
Common plans include:
- UnitedHealthcare (plans vary)
- Cigna
- MetLife
- Superior Dental
- Medical Mutual
- Luminare
Out-of-Network Plans Requiring Upfront Payment
For the following plans, we may collect payment upfront and submit the insurance claim on your behalf. Your insurance company will then send any eligible reimbursement directly to you.
- Aetna Dental Plans
- GEHA
- TruAssure
- United Concordia
- Aetna Medicare
- Humana Dental
- Connection Dental
- DHA / Sun Life
Plans We Are Unable to Bill
Some Medicaid and Medicare plans only provide benefits when you visit a participating in-network provider. Because we are not participating providers with the plans below, visits at our office would be self-pay.
Our Membership Plan may be an option for patients with these plans who would still like to receive care at our office.
- Mount Carmel MediGold (Medicare)
- AmeriHealth
- AmeriHealth Caritas
- Molina
- Buckeye Community Plan / Buckeye Health Plan
- Humana Medicaid
- CareSource Dual Advantage
- UnitedHealthcare Medicaid
Insurance participation and benefits can change. Coverage is determined by your individual plan, and we recommend confirming your benefits directly with your insurance carrier.
Insurance FAQ
Do you take my insurance?
We can bill most PPO dental insurance plans and apply your in-network or out-of-network benefits. If we are not contracted with your plan, we recommend checking with your insurance company to confirm your out-of-network benefits.
What does in-network vs. out-of-network mean?
In-network means we have a contracted fee agreement with your insurance plan. Out-of-network means we are not contracted with your plan, but you may still have benefits you can use at our office. Coverage varies by plan, so we recommend confirming your out-of-network benefits with your insurance company.
Can I join the membership plan if I have insurance?
Our membership plan is generally for patients without traditional dental insurance and cannot be combined with PPO dental insurance.
An exception may apply if you have a Medicaid or Medicare plan that our office is unable to bill. Contact us and we can help determine if the membership plan is an option for you.
I have Medicaid and cannot find a provider who takes my plan. What should I do?
Call your insurance company and ask for a current list of participating dental providers in your area. They may have additional providers that do not appear online.
If you still cannot find care, you can also ask whether you are eligible to switch to a different Medicaid plan with more providers in your area.
Why do I have to pay upfront for my out-of-network plan?
Some out-of-network plans will not tell us exactly what they will pay until your claim is processed. In these cases, we may collect the full fee upfront and submit the claim for you so your insurance company can reimburse you directly.
This keeps things clearer upfront and helps avoid an unexpected bill or credit from our office months later.
My insurance is maxed out, but I still want treatment. What are my options?
You don't have to put your dental care on hold just because your benefits are maxed out! We offer financing through Sunbit and CareCredit, which may allow you to spread your treatment cost into monthly payments, subject to approval.
